Social Work

The Social Work Program at Christ Community Health Services is an important addition to the services offered within the primary care needs of our patients.  Social Workers (Bachelor and Master level Social Workers) as well as Social Work interns address the Social Service needs of our patients.

Patients are offered the opportunity to meet with a Social Worker during the patient’s visit or consult with a Social Worker by phone.  Social Workers assess for the needs such as Food Insecurity, Clothing Needs, Housing Issues, Transportation needs, Medication Assistance Information, Employment Resources and much more. In addition, Social Workers assist our patients by connecting them with resources offered within our community. Social Work Services are offered to patients who see providers at all of our Christ Community clinics.
