The Orange Hype intervention study includes home-based care as a part of a team-based care approach to healthcare delivery using Community Health Workers (CHWs) in Orange Mound. The intervention extends delivery of hypertension prevention and treatment to include those not reached by traditional delivery systems, meeting residents where they reside, by providing home-based blood pressure screenings, medication administration, health education, and spiritual counseling. The intervention provides a more intense approach to hypertension management compared to the traditional clinic setting. By utilizing CHWs, the intervention engages the community, improves community relations, and connects residents to needed healthcare and social support. The intervention is supported by physicians and Christ Community Health Services Clinic that resides in the Orange Mound Community.
