1. Where is Christ Community Health Center located?
Christ Community has 6 health centers, 5 dental centers and 5 pharmacies located within communities that are medically underserved by primary care physicians.

Christ Community is a Christian health facility that offers adult and pediatric care.
3. What services are offered by Christ Community Health Services
All Christ Community Health Centers offer: Pediatric and adult Primary care, Women’s health, HIV Treatment, Spiritual counseling.

The Raleigh, Frayser, Broad Ave., Hickory Hill, South Memphis and Mobile, locations offer
The Broad, Frayser, Third, Hickory Hill and Orange Mound centers offer Pharmacy Services
The Broad Frayser, Hickory Hill, Third Street and Raleigh locations offer Dental Services
4. Do I need an appointment?
Yes. Appointment lines open at 7:30 am for same day and new patient appointments at 7:30 am.
Established patients who have a history of keeping appointments and arriving on time have the ability to schedule up to 2 weeks in advance.
5. Is there a no show policy?
Yes. We ask that patients that need to miss an appointment contact our health center immediately. If 2 appointments are broken, you will need to speak with a CCHS representative prior to scheduling any future appointments. At 4 missed appointments you will be dismissed from receiving services at CCHS.
6. Do you accept insurance?
Yes. We accept most major commercial insurance plans as well as TNCare, Medicare and Medicaid.
7. Can I get treatment if I don’t have insurance?
For patients who do not have healthcare insurance and who can provide proof of income, we offer a number of discount programs with fees ranging from $25-$80.
8 Does that include my lab fees?
CCHS has an arrangement with LabCorp to offer discounts to our patients based upon income and household size. Information about lab discounts will be provided at the first visit.
9. What should I bring to my appointment?
Please bring a photo ID, your insurance card (if you have insurance) your co-pay and any medications you are taking. If you are bringing a child please bring their vaccination records to each visit.
10. Do you offer late night or after hour appointments?
Most of our health centers are open one evening a week and on weekend a month. Please check with your health center regarding their evening and weekend clinics
