Participating Churches

Currently, the program had partnerships with numerous churches, as well as agencies, groups, and organizations that share a special concern for the health of African-Americans. The participating churches represent almost 50,000 individuals.

 Activity Reporting Forms  - Must be recieved the 2nd Monday of the                                                  following month

 Event Form -                      Must be recieved at least 4 - 6 weeks                                            before event.
